REQUEST AND RETURN MATERIALS THROUGH APS MAIL!
We're going to take a page out of the Post Office's book and allow our patrons to return materials they checked out in an APS mailing envelope. We only ask that the items fit EASILY and SECURELY into the envelopes before they are sent back to us. Larger items should be packaged in a STURDY CARDBOARD BOX that is no larger than a case of paper. It should be packaged with sufficient packing materials in the box to prevent damage to the materials. The box should be taped closed or securely wrapped with string or twine. The box should be CLEARLY addresed to:

Patrons can search our catalog online at www.unifiedlibrary.com and check out materials by emailing or calling us. If the material will fit into an APS mailer, we will send it to you that way. Otherwise, we will ship the materials in a box no larger than a case of paper. Items larger than a case of paper will have to be picked up. We will let you know about larger items and hold them for pick up for one week.

When you request materials from us by email or phone message, please include your name, telephone number and employee number.

We will trying the delivery system for a few months. We will continue to circulate materials this way if it is successful.

SUMMER SCHOOL CHECK OUTS
Summer school check outs begin June 10. Check outs are limited to 10 items. We will be open our regular hours during the summer. All materials will be due July 23.

2010-2011 SCHOOL YEAR CHECK OUTS
We will begin check outs for the 2010-2011 school year on August 11. See our announcement about requesting and returning materials through the APS mail.

OVERDUE MATERIALS AND FINES
If you have any overdue materials or fines you will not be able to check out any new items until these issues are resolved. There may be a temporary 5 item limit on your account.

AUDIO TEXTBOOK INFORMATION
Special education teachers who order audio textbooks for their eligible students can download the necessary forms by clicking on the "Audio Textbook Form" button on the left side of this page. A six page PDF document will appear. You can print all of the pages for your information, but the only ones that need to be filled out and returned to Sue Lacey at the Unified Resources Library are pages 2, 3, and 4. The last page, which is the "Written Request for Alternate Format Instructional Materials," is optional. You may fill out two copies and mail one directly to the publisher and send the other one to Margaret Montano at the Regional Instructional Materials Center (RIMC) at Montgomery Complex.

Please make sure you fill out the forms accurately and completely. Incomplete forms cause delays in ordering materials and will be returned.
